Referral Services Our case management team works very hard to ensure patients receive the utmost benefit. Ongoing patient assessment helps to identify specific patient needs. Any patient need is reported to the Director of Nursing. The Director of Nursing in consultation with other team members will refer the patient to the relevant service. |
